What you'll do

  • Conduct virtual adult and/or pediatric wellness visits, health assessments, and preventive care interventions.
  • Review caseload and prepare technology and charting tools at the start of the day.
  • Coordinate with in-home clinical team members (Sprinters) to evaluate vital signs, arrange blood draws, and address patient needs.
  • Perform medication reviews, cognitive screenings, and patient education.
  • Provide comprehensive care coordination and management, including referrals and documentation of findings.

What they require

  • Board Certified as a Family Nurse Practitioner
  • Active Family Nurse Practitioner License in Texas
  • Pass national background check and valid clinical license search
  • Consistently exhibits professionalism, integrity, accountability, confidentiality, care and compassion
  • Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA secure messaging apps
