What you'll do

  • Conduct virtual adult and/or pediatric wellness visits and health assessments.
  • Begin each day by reviewing caseload and preparing required technology.
  • Perform health risk assessments, medication reviews and cognitive screenings.
  • Partner with in-home Sprinter team members to evaluate vital signs and arrange blood draws.
  • Provide comprehensive care coordination, referrals, medication management and documentation of findings.

What they require

  • Board Certified as a Family Nurse Practitioner.
  • Active Family Nurse Practitioner License in Michigan.
  • Pass national background check and valid clinical license search.
  • Strong written and verbal communication skills; able to work independently or in a team.
  • Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA secure messaging apps.
