The ideal candidate will excel in acute and chronic psychiatric care, delivering high-quality mental health services to patients in various settings. This role involves rounding in the BHUs and providing support services to the ED and Med-Surg floors, as well as supporting the PACE program. Bilingual (Spanish) required. Develop and manage individualized treatment plans, including medication management and therapeutic interventions. Collaborate with the interdisciplinary PACE team, including physicians, nurses, social workers, therapists, and the collaborating psychiatrist, to ensure coordinated participant care. Provide ongoing psychiatric follow-up, crisis intervention, and behavioral health support across care settings. Educate participants and caregivers on mental health conditions, medications, and treatment options. Maintain timely, accurate documentation and participate in quality improvement initiatives to enhance behavioral health services within the PACE program.
