What you'll do

  • Provide care coordination and clinical oversight for assigned patient caseloads across both agencies, working directly with physicians, healthcare providers, and community partners.
  • Review, audit, and sign clinical documentation, including Starts of Care (SOC), Recertifications, Resumptions of Care (ROC), Discharges, and external physician orders.
  • Serve as the point of contact for incoming clinical inquiries from field staff and patients, offering guidance and escalating complex issues to the DPCS as necessary.
  • Analyze clinical documentation and workflows to identify quality improvement opportunities and ensure alignment with agency policies and regulations.
  • Support remote onboarding, training, and clinical guidance for new and existing field clinicians as needed.

What they require

  • Licensure: Active, unrestricted California Registered Nurse (RN) license
  • Home Health Experience: Minimum of 1-2 years of hands-on home health RN experience
  • Supervisory Experience: Minimum of 1 year of supervisory experience in home healthcare or a similar field
  • Remote Capability: Ability to fulfill the duties of this role remotely, with strong digital communication skills.
