The RN collaborates with an interdisciplinary care team to identify and close gaps in preventive care, chronic disease management, and regulatory requirements. This role supports the delivery of high-quality, efficient, and cost-effective healthcare services by providing patient education, promoting self-management, supporting evidence-based care plans, and coordinating transitions of care. Education Qualifications: Educational Requirements: BSN or ADN degree from an accredited school or college of nursing Licensure/Certification Qualifications: Certification/Licensure Requirements: Current RN license in the state of North Dakota, Minnesota, and/or Wisconsin upon hire. Ability to obtain RN license in all 3 states within 3 months of hire.
