What you'll do

  • Works with patients identified as at-risk due to substance use disorders and related medical or behavioral health needs.
  • Collaborates with an interdisciplinary care team to identify and close gaps in preventive care, chronic disease management, and regulatory requirements.
  • Supports the delivery of high-quality, efficient, and cost-effective healthcare services by providing patient education, promoting self-management, supporting evidence-based care plans, and coordinating transitions of care.

What they require

  • BSN or ADN degree from an accredited school or college of nursing
  • Current RN license in the state of North Dakota, Minnesota, and/or Wisconsin upon hire.
  • Ability to obtain RN license in all 3 states within 3 months of hire.
  • FTE Flex Days (United States of America)
  • Shift Start Time: 0800 (flexible on the start and end time)
  • Shift End Time: 1500
