Resource Specialist Care Manager

Who We Are Imagine Pediatrics is a tech enabled, pediatrician led medical group reimagining care for children with special health care needs. We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity. Candidate must be licensed in the one of the following states: NY, NJ, NC, GA, FL, TX, MO, TN or Washington DC. 10 % travel for offsite meetings, training, and educational purposes. 8:00am-5:00pm EST Monday-Friday with 2 days determined with supervisor being 10:00am-7:00pm EST. What You’ll Do As a Resource Specialist Care Manager, you will work with the families of medically complex children providing emotional support, care coordination, and resourcing support in navigating the healthcare system. You leverage an integrated technology platform and are complimented by an entire interdisciplinary team including physicians, RNs, care team assistants, mental health therapists, community health navigators, pharmacists, dietitians, and more. In this position, you will: Conduct thorough assessments, exploring the current medical, behavioral, and complex social needs. Identify barriers to care, such as food insecurity, unstable housing, transportation challenges, and insurance limitations. Collaborate with families and multidisciplinary teams to establish individualized care goals. Develop and maintain an integrated care plan addressing medical, behavioral, and social needs. Ensure timely follow-up on referrals, evaluations, and treatment recommendations. Offer education to families about navigating local resources and agencies. Provide emotional support and guidance when families are facing challenging situations. Build safety plans for interpersonal/domestic violence and caregiver health needs. Advocate for resources and removal of barriers and connect families with available resources. Build and maintain relationships with community organizations that serve children with medical and behavioral health complexities. Stay informed on emerging programs and resources relevant to the population served. Represent the organization at community events, health fairs, and resource meetings. Participate in ongoing scheduled consultations with an interdisciplinary team to monitor patient progress. Perform other duties as assigned. What You Bring & How You Qualify First and foremost, you’re passionate and committed to reimagining pediatric health care and creating a world where every child with special health care needs gets the care and support they deserve. In this role you will need: Masters’ degree in social work required LMSW or comparable state licensure required 2-3+ years postgrad experience in social service case management required Minimum 1 year of experience working with pediatric population required Knowledge and application of motivational interviewing practices and/or techniques Comfortability working with acute populations and familiarity with school and community resources highly preferred Experience with providing telehealth services and engaging with families in a remote environment Bilingual Spanish strongly preferred What We Offer (Benefits + Perks) The salary range for this position ranges from $70,000-77,000 annually in addition to competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable).
