What you'll do

  • Serve as the primary clinical point of contact for patients navigating their remote care journey.
  • Lead clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure, as well as adherence to evidence-based guidelines for type 2 diabetes and hypertension.
  • Oversee patients, manage clinical escalations, and collaborate closely with the patient’s physician or provider to optimize treatment plans, adjust medications, and determine the appropriate level of care.
  • Manage a panel of patients receiving remote care, making timely clinical decisions based on evidence-based guidelines for heart failure, hypertension, and type 2 diabetes.
  • Deliver high-quality, evidence-based care by following established clinical protocols and workflows while applying sound clinical judgment to individual patient needs.
  • Initiate and titrate guideline-directed medical therapy (GDMT) while collaborating closely with physicians, RNs, and care teams to optimize outcomes.
  • Respond promptly to clinical escalations generated through remote patient monitoring, including abnormal vitals, symptoms, and laboratory results.
  • Provide both synchronous and asynchronous patient care, including reviewing patient-generated health data, responding to remote patient monitoring alerts, communicating through secure messaging, and conducting scheduled virtual visits when appropriate.
  • Deliver safe, efficient, high-quality care while consistently meeting established productivity and documentation expectations.
  • Adapt quickly to evolving workflows, software enhancements, and operational processes as we continuously improve our care model.
  • Use multiple technology platforms confidently and provide feedback that helps improve our clinical operations and technology.
  • Collaborate across clinical and operational teams to identify opportunities to improve patient care, efficiency, and scalability.

What they require

  • Active Compact Multi-State RN license.
  • Active NP licensure in at least two of the following states: TX, MI, NC, CA, PA, NJ, CT, AL.
  • Exceptional candidates with fewer licenses may be considered based on overall qualifications and ability to obtain additional priority state licenses.
  • Licenses must be active and in good standing at time of hire.
  • Monday through Friday schedule (Flexible scheduling available, with a core shift schedule of 9:00 AM to 6:00 PM).
  • High level of clinical autonomy, strong judgment, and close collaboration with Registered Nurses and Patient Success teams to deliver timely, patient-centered, and scalable remote care.

  • 5+ years experience treating patients with chronic diseases (CHF, Hypertension, T2D) either in an outpatient or inpatient setting at a high-performing medical center.
  • 3+ years of cardiology experience with a focus on heart failure management and guideline-directed medical therapy (GDMT).
  • Active ANCC or AANP certification.
  • Master’s Degree as a Nurse Practitioner with the willingness to expand state licensure as Cadence adds new markets and partners.
  • Home office setup with consistently stable Wi-Fi with strong upload and download speeds.
  • A Wi-Fi speed test is required during the interview process to verify that these standards are met.
  • Demonstrated success working in fast-paced environments where priorities, workflows, and technology evolve frequently.
  • Comfortable receiving frequent feedback and continuously adapting processes.
  • Highly organized with excellent time management and the ability to balance patient care with productivity expectations.
  • Self-directed and accountable while working remotely with minimal supervision.
  • Curious, resilient, and energized by helping build new ways of delivering care.
  • Preferred: Experience working in a CHF bridge clinic or T2D clinic, a startup environment, and working with remote patient monitoring technology.
  • Comfortable using AI-assisted clinical tools (such as Gemini, Open Evidence, or comparable platforms) while maintaining clinical judgment as the primary guide for every patient decision.
