What you'll do

  • Take cases that reach you already triaged: our healthcare ops team confirms the essentials first — insurance, needs, level of urgency, clinical history — then assigns the case to you
  • Read the case and build an execution plan: what has to happen, in what order, how fast
  • Work the plan using the resource directories we give you — and when they're not enough, go find your own
  • Keep ops in the loop as you go: progress, blockers, what you need to move faster
  • Call the patient — to share progress, to ask about anything you're unsure of, and so they know someone is fighting for them
  • Document every interaction and support activity in our platform, accurately and as you go, so handoffs across a 24/7 team are seamless
  • Maintain confidentiality and comply with HIPAA, privacy, and all applicable platform standards

What they require

  • An active LPN/LVN license; or Community Health Worker (CHW) certification; or equivalent community health / patient-navigation experience.
  • 2+ years of patient- or community-facing support experience (care coordination, case management, member advocacy, health coaching, benefits navigation, or similar).
  • Working knowledge of the U.S. healthcare system and insurance.
  • Comfort working independently, managing multiple urgent cases at once, and maintaining strong documentation.
  • Reliable computer, internet, and a quiet space for patient calls.

Benefits

  • Remote work
  • Flexible hours (20-40 hours/week)
  • Opportunity to work up to 40 hours per week
