Full-time remote obesity medicine physician or endocrinologist with MD/DO, board certification, ABOM diplomate or Endocrinology board certification. Must be licensed to practice and prescribe in CT, DE, ME, NH, NJ, PA, RI, or VT and hold an IMLC Letter of Qualification. Telemedicine experience, payer credentialing, and collaboration with dietitians/APPs are expected.

What you'll do

  • Provide direct patient care in a clinical setting, focusing on evidence-based obesity treatment and chronic weight management.
  • The physician will have their own panel and will collaborate with Registered Dietitians.
  • In the future, this physician role will be responsible for chart reviews and conduct return visits for APP patients every third visit to ensure quality and continuity of care.
  • Provide treatment planning and medical management support for complex cases or escalated concerns.
  • Prescribe non-controlled obesity medications for patients under direct care.
  • Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team.
  • Assist in the professional development of APPs by fostering clinical growth, offering feedback, and supporting continuing education.
  • This role could include APP collaboration and supervision responsibilities.
  • Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.
  • Attend relevant departmental meetings and contribute to quality improvement efforts within the obesity medicine program.
  • Participate in a rotating after hours call schedule with Form Health clinical leadership.

What they require

  • MD or DO degree with board certification; must be an American Board of Obesity Medicine (ABOM) diplomate or board certified in Endocrinology.
  • Licensed to practice and prescribe legally in CT, DE, ME, NH, NJ, PA, RI, or VT.
  • Must hold an LOQ (Letter of Qualification) with the IMLC (Interstate Medical Licensure Compact).
  • Are or have in the past credentialed with insurance payers.
  • Have excellent interpersonal skills for patient and team interactions.
  • Are committed to the highest quality of medical care and an evidence-based approach to obesity management.
  • Have prior experience conducting telemedicine visits and who have a great "web-side" manner.
  • Have prior experience working collaboratively with advanced practice professionals and registered dietitians.
  • Preferred: Strong preference for multiple state licenses.
  • Preferred: Bilingual candidates with medical fluency in Spanish are highly preferred.
