Physical Therapist with an active California PT license, current CPR certification, valid driver’s license, insured vehicle, and at least 2 years of PT experience. Home health experience is a plus. Per diem role serving patients in California home health settings.

What you'll do

  • Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
  • Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
  • Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
  • Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
  • Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
  • Manage documentation including clinical progress summaries and discharge planning.
  • Identify needs for additional home health services and coordinate referrals as appropriate.
  • Perform other duties as required for patient services.

What they require

  • Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
  • Active California Physical Therapy license.
  • Minimum 2 years of PT experience; home health background is a plus.
  • Strong communication skills and the ability to connect with patients and families.
  • Current CPR certification with AHA.
  • Valid driver’s license and reliable, insured vehicle.
