Per Diem RN (Triage) — remote role restricted to Florida. Requires a current Florida or multistate RN license (or ability to obtain within 30 days) and minimum 1 year nursing experience; experience with EMR/EHR and telephone triage. Responsible for telephonic patient triage, documentation in EMR, and coordination of in-person visits as needed.

What you'll do

  • Triages after-hours phone calls and ensures callers’ needs are met efficiently using excellent customer service skills.
  • Provides telephone advice and education for patient needs and reassurance to patients and families.
  • Accurately enters and maintains patient information in the electronic medical record (EMR) and tracks patient referral files.
  • Coordinates in-person visits when needed or requested and communicates with the Interdisciplinary Group (IDG).
  • Utilizes appropriate support/expert resources to resolve complex situations and documents patient/family contact information.

What they require

  • Current Florida RN license OR current active single-state out-of-state nurse license OR current active multistate/compact nurse license; ability to apply for/obtain multi-state RN license within 30 days if not already a holder.
  • Minimum of one (1) year nursing experience; hospice or hospital experience preferred (Preferred: hospice or hospital experience).
  • Previous experience working with an EMR/EHR system.
  • Ability to effectively communicate in English (verbal and written).
  • Consent to drug and/or alcohol testing after a conditional offer and submission through the Florida Care Provider Background Screening Clearinghouse as required; ability to perform essential physical requirements including lifting, bending, pushing/pulling, kneeling.
