What you'll do

  • Conduct comprehensive clinical assessments and oversee member health
  • Facilitate end-of-life care discussions
  • Actively engage with members, addressing clinical issues and managing escalations
  • Support members throughout the oncology care continuum, from screening to survivorship or end-of-life care
  • Coordinate care and provide clinical support
  • Identify and address member needs with care coordination, symptom management, nutritional support, discharge planning, and provider referrals
  • Participate in case conferences to monitor member progress and collaborate on support plans
  • Foster partnerships with payers and providers to optimize care delivery and minimize readmissions
  • Collaborate with non-clinical team members on social determinants of health needs
  • Be available for urgent clinical escalations and provide clinical consult support

What they require

  • Active, unrestricted compact Registered Nurse (RN) license required
  • Degree in Nursing (ADN or BSN) required; BSN preferred
  • Compact + single state licensure in New York and/or Illinois preferred
  • At least 5 years of nursing experience, including 3 years in solid tumor oncology nursing
  • Oncology-related certifications such as OCN, AOCN, AOCNS, or CCM required or obtained within 2 years of hire
