What you'll do

  • Provides fully remote virtual serious illness care management to patients and families across home-based, facility-based, and inpatient hospice; palliative care; PACE, and other service lines.
  • Consults with medical staff and interdisciplinary team to ensure optimal patient care.
  • Performs virtual assessment of patient/family/caregiver needs.
  • Identifies, manages, orders, and prescribes medication and equipment as appropriate.
  • Adjusts and supports plan of care according to Chapters policies and nurse practice act.
  • Assesses caregiver ability and provides education to increase caregiver confidence.
  • Answers clinical questions, enters orders and prescriptions, and supports clinical needs of patients and families.
  • Maintains access to EMR, electronic prescribing systems, and required websites/applications and documents interactions timely.
  • Communicates and collaborates with primary team to ensure continuity of care.
  • Supports discharge planning including prescribing medications and entering discharge orders and instructions.
  • Participates in Quality Assurance/Performance Improvement activities including medical record and peer reviews.
  • Keeps knowledge and skills current and obtains/maintains hospital/facility credentialing as required.

What they require

  • Minimum of 3 years of recent clinical experience, including substantial experience in hospice, palliative care, and/or acute inpatient care.
  • Masters or Doctoral degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
  • Must hold national certification, licensure, and prescriptive authority for advanced nursing practice in their home state and in additional state(s) where care is provided; licensure and prescriptive authority must be obtained and maintained for all required jurisdictions.
  • Applications for required licenses must be submitted within 60 days of hire/notification.
  • Must hold and maintain Basic Life Support certificate from an approved organization.
  • Must hold a compact RN license in their home state by date of hire.
  • Schedule includes nights, evenings, weekends, and/or holidays as needed.
  • Consent to drug and/or alcohol testing after conditional offer and ongoing compliance with Drug-Free Workplace Policy.
  • Employees performing services for Florida affiliates are submitted through the Florida Care Provider Background Screening Clearinghouse to verify eligibility after conditional offer and ongoing eligibility.
