What you'll do

  • Provides fully remote virtual serious illness care management to patients and families throughout the continuum of care (home-based, facility-based, inpatient adult and pediatric hospice, palliative care, PACE, and additional service lines).
  • Consults with medical staff and/or other members of the IDT as indicated to ensure optimal patient care.
  • Performs virtual assessment of patient/family/caregiver needs.
  • Identifies, manages, orders, and prescribes medication and equipment as appropriate.
  • Adjusts and supports the plan of care in accordance with Chapters policies and nurse practice act guidelines.
  • Assesses caregiver’s ability to meet patient needs and provides education to increase caregiver confidence.
  • Supports staff by answering clinical questions, entering orders and prescriptions, and supporting clinical needs of patients and families.
  • Maintains access to EMR, electronic prescribing systems, and other required websites/applications and documents interactions timely.
  • Communicates and collaborates with the primary team to ensure continuity of care.
  • Supports discharge planning including prescribing medications and entering discharge orders and instructions.
  • Participates in Quality Assurance/Performance Improvement activities, including medical record and peer reviews.
  • Keeps knowledge and skills current to assure quality patient care and professional growth.
  • Obtains and maintains hospital/facility credentialing as required and participates in departmental and medical staff activities.
  • Provides patient and family education and functions as a member of the interdisciplinary team.
  • Provides teaching to staff and acts as a role model in customer service.
  • Demonstrates flexibility in providing coverage and/or availability.

What they require

  • Must have a minimum of 3 years of recent clinical experience, including substantial experience in hospice, palliative care, and/or acute inpatient care.
  • Must hold a Masters or Doctors degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
  • Must hold national certification, licensure, and prescriptive authority for advanced nursing practice in their home state and in additional state(s) where care is provided; licensure and prescriptive authority must be obtained and maintained for all required jurisdictions.
  • Applications for required licenses must be submitted within 60 days of date of hire/notification.
  • Must hold and maintain Basic Life Support certificate from an approved organization and compact RN license in their home state by date of hire.
  • Consent to drug and/or alcohol testing after a conditional offer and ongoing compliance with Drug-Free Workplace Policy.
  • Employees performing services for Florida affiliates are submitted through the Florida Care Provider Background Screening Clearinghouse after a conditional offer and ongoing eligibility verification.
