Part-time remote Chronic Care Coordinator for Michigan; must be an RN/LPN with an active Michigan license. Independent 1099 contractor (potential to convert to W2) working ~20–30 hours/week. Productivity-based pay per completed patient encounter ($10.00–$16.00 per encounter depending on volume).

What you'll do

  • Perform telephonic encounters with patients on behalf of partners each month.
  • Develop detailed care plans within care plan templates in the electronic health record.
  • Abide by the plan of care and orders of the practice and provide prevention/intervention through motivational coaching.
  • Create billable Chronic Care Management episodes and improve visits for chronic condition management.
  • Attend regularly scheduled (mandatory) meetings such as bi-monthly staff meetings and monthly one-on-ones.

What they require

  • Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.).
  • Current active license to practice as an RN/LVN/LPN held in the state of MICHIGAN (NOT A COMPACT STATE) with no disciplinary actions.
  • Minimum of two (2) years clinical experience in Med/Surg, Case Management, and/or home health care.
  • Hands-on experience with Electronic Medical Records and familiarity with Windows desktop and Microsoft Office 365 (Teams, Excel).
  • HIPAA-compliant home workspace, high-speed internet, and a desktop or laptop computer (Windows or Mac); NO Chromebooks OR iPads.
