Remote LVN Case Manager with an active unrestricted CA LVN license and 2+ years of care management experience supporting vulnerable populations. Must reside in AZ, CA, CO, FL, IL, NV, NC, OR, TX, UT, WA or GA (full list per post: AZ, CA, CO, FL, GA, IL, NV, NC, OR, TX, UT, WA). Must work Pacific Time Monday–Friday 8:30am–5:00pm.

What you'll do

  • Assess member needs in physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referral/linkage to community services
  • Oversee the development of client care plans and goal setting
  • Offer services where the member resides, including office-based, telehealth, or field-based
  • Connect clients to social services and supports
  • Advocate on behalf of the client with health care professionals
  • Utilize evidence-based practices (Motivational Interviewing, Harm Reduction, Trauma-Informed Care)
  • Conduct outreach and engagement to link client to ECM program and log in CRM
  • Evaluate client’s progress and update SMART goals
  • Provide mental health promotion
  • Arrange transportation (e.g., ACCESS)
  • Complete all documentation and outcome measures within timelines
  • Maintain patient records in EMR and other business systems
  • Complete monthly reporting for program compliance
  • Attend assigned training

What they require

  • Active, unrestricted Licensed Vocational Nurse (LVN) license in California required
  • Willing to work Monday-Friday 8:30am-5:00pm Pacific Time, with possibility of evenings and weekends
  • 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
  • Working knowledge of government and community resources related to social determinants of health
  • Excellent oral and written communication skills
  • Positive interpersonal skills
  • Must have general computer skills and a working knowledge of Google Workspace, MS Office, and the internet
