What you'll do

  • Accurately document patient/member encounters in Thyme Box and supported EHRs, capturing histories, assessments, treatment plans, orders, diagnostic results, referrals, prescriptions, and follow-up instructions.
  • Organize patient charts and gather relevant clinical information ahead of visits, including vital signs, medication reconciliation, and other required data.
  • Review member eligibility for services and verify insurance/coverage.
  • Facilitate referrals, follow-up appointments, and diagnostic testing and keep care plan information updated to ensure continuity of care.
  • Conduct chart reviews across multiple electronic platforms to consolidate and streamline transitions of care information.
  • Monitor shared clinical inboxes and electronic communications, triaging and routing messages promptly to the appropriate provider, department, or team.
  • Coordinate and communicate with internal and external healthcare partners to support seamless and coordinated care.

What they require

  • At least 2 years as a LPN
  • Compact LPN license
  • Skilled in juggling multiple tasks and working under pressure without sacrificing organization in your communications and documentation.
  • Effective listener and communicator.
  • Build rapport and excellent working relationships with members and colleagues.
  • Comfort with ambiguity.
  • Understand that rapid changes to the business, strategy, organization, and priorities are par for the course… and part of the adventure.
  • A desire to learn how to use new technologies.
  • Identify priorities and take action.
  • Know how to identify and prioritize a member's needs and do what it takes to address urgent needs immediately.
