Licensed Practical Nurse (LPN) for intake in Home Health and/or Hospice. Requires an LPN license in the state of service or through eNLC. Preferred candidates have 2+ years of experience in Home Health/Hospice or utilization review.

What you'll do

  • Manage the intake process for new patients, including receiving referrals from healthcare providers, insurance verification, and scheduling initial assessments.
  • Serve as the primary point of contact for patients, families, healthcare professionals, and referral sources regarding Home Health and/or Hospice services.
  • Ensure accurate and timely documentation of patient information, including medical history, assessments, care plans, and communication with other healthcare providers.
  • Collaborate with interdisciplinary team members to develop individualized care plans based on patients' needs, goals, and preferences.
  • Coordinate services and resources needed to support patients' care plans, such as medical equipment, supplies, therapy services, and community resources.
  • Provide education to patients and families about their condition, treatment options, self-care techniques, and available support services.
  • Monitor patients' progress and response to treatment, reassessing needs as necessary, and adjusting care plans accordingly.
  • Coordinate transitions between care settings, such as hospital discharge to Home Health and/or Hospice services, ensuring continuity of care and smooth transitions.
  • Participate in quality assurance activities, such as audits, reviews, and performance improvement initiatives, to ensure compliance with regulatory standards and quality outcomes.
  • Collaborate with healthcare professionals, including physicians, nurses, therapists, and social workers, to ensure coordinated and comprehensive care for patients.

What they require

  • Associates Degree or Certification in Nursing
  • Current Licensed Practical Nurse license issued by the state in which services will be provided or current multi-state Licensed Practical Nurse license through the enhanced Nurse Licensure Compact (eNLC).
  • Preferred: Bachelor’s degree in nursing.
  • Preferred: Two (2) years of Home Health and/or Hospice experience.
  • Preferred: Two (2) years of experience in utilization review/chart audit.
