Baba is seeking an experienced and compassionate Healthcare Advocate with Care Advocacy Experience to join our fast-growing care navigation and support team. If you're passionate about helping seniors, caregivers, and their families navigate the complexities of healthcare and aging, Baba is the place to make an impact with purpose.

What you'll do

  • Develop trusting relationships with patients, grounded in listening, respect, and clarity.
  • Assess the whole person — medical, social, and environmental factors — to pinpoint barriers and needs.
  • Create individualized care plans that integrate medical, social, and behavioral supports.
  • Support patients across a range of needs, including: Connecting to local services (transportation, meal delivery, home care, social supports), Analyzing medical bills and helping with claim appeals or explanations, Identifying medication cost savings, co-pay assistance, or formulary options, Linking to social support networks (caregivers, community programs, peer groups), Organizing and maintaining medical records, care histories, and provider communication, Coaching and supporting patients on best practices for managing conditions and navigating care.
  • Document interactions, observations, and patient progress in Baba’s systems with clarity and timeliness.

What they require

  • Associate's or Bachelor's degree in Nursing, Social Work, Public Health, Health Administration, or a related healthcare field OR Certificate/Diploma in Practical Nursing (LVN/LPN).
  • Preferred: Active RN, LSW/LMSW/LCSW, CCM/ACM, CHW, LPN/LVN, or other relevant clinical or care management credential.
  • 2+ years of experience in care navigation, patient advocacy, care management, case management, home health, discharge planning, utilization management, or clinical social work, preferably supporting older adults or Medicare populations.
  • Solid knowledge of Medicare rules, eligibility, benefit structures, claims, and appeals.
  • Deep understanding of Social Determinants of Health (SDOH) and experience working with diverse and underserved populations.
