If you want autonomy, scale, and the ability to grow your earnings, keep reading. ______________________________________________________________________________________________ The Opportunity You’ll manage a large panel of Medicare patients with chronic conditions, delivering monthly clinical calls that drive real outcomes and reduce hospitalizations. This role is built for nurses who can operate independently, manage complexity, and execute consistently at a high level.
