Remote U.S.-based 1099 Care Advocate for Medicare patient navigation, 20–40 hours/week. Requires CHW certification or equivalent patient-navigation/community health experience, 2+ years patient/community-facing support, and working knowledge of U.S. healthcare and Medicare.

Spanish Preferred / bilingual; Spanish or another language common among Medicare patients.

What you'll do

  • Serve as the primary point of contact for patients after their intake visit.
  • Help patients understand and act on their care plan, including scheduling appointments, finding in-network providers, and using Medicare benefits.
  • Resolve navigation friction involving portals, referrals, appointment logistics, forms, and provider communications.
  • Identify practical and social-needs support such as food, housing, and transportation, and connect patients to available resources.
  • Coordinate with clinical team members involved in a patient's care while providing non-clinical support only.
  • Document every patient interaction and support activity in the platform accurately and on time.
  • Maintain confidentiality and comply with HIPAA, privacy, and all applicable platform standards.

What they require

  • Genuinely care about older adults and people facing barriers to care, with patience, warmth, and respect in every interaction.
  • Organized and self-directed, able to prioritize across a full caseload, follow through, and keep clean documentation without being chased.
  • Comfortable with technology and able to learn new tools quickly, including EHRs, CRMs, messaging platforms, and scheduling platforms.
  • Strong communicator who can build trust over the phone and explain confusing things simply.
  • Stay calm in ambiguity and take initiative to solve problems.
  • Community Health Worker certification or equivalent community health / patient-navigation experience.
  • 2+ years of patient- or community-facing support experience, such as care coordination, case management, member advocacy, health coaching, benefits navigation, community health work, or similar.
  • Working knowledge of the U.S. healthcare system and Medicare.
  • Comfort working independently, managing a caseload, and maintaining strong documentation.
  • Reliable computer, internet, and a quiet space for patient calls.
  • Ability to commit at least 20 hours per week, with opportunity to work up to 40 hours per week.
  • Generally available during core business hours, 9:00 a.m.–5:00 p.m. ET, Monday–Friday.
  • Preferred: Experience supporting older adults or patients with complex or serious illness.
  • Preferred: Familiarity with Medicaid and other public programs.
  • Preferred: Bilingual in Spanish or another language common among Medicare patients.
  • Independent contractor responsible for own taxes; this is not a benefitted role.
