What you'll do

  • Be the primary point of contact for Wellthy families across the US, guiding them through caregiving complexities
  • Manage logistics of care including coordinating medical appointments, researching providers, tracking medications, managing medical bills, and assisting with insurance and facility vetting
  • Develop comprehensive care plans and oversee ongoing needs of families
  • Collaborate with Care Advisers to ensure seamless, high-quality service
  • Take ownership of member-facing relationships ensuring timely and thorough execution of priorities
  • Proactively identify and implement process improvements to enhance care delivery and member outcomes
  • Mentor and provide guidance to team members; support knowledge-sharing initiatives
  • Represent Wellthy externally at events such as health fairs, webinars, or open enrollments
  • Contribute to team culture and cross-functional initiatives

What they require

  • Bachelor’s degree required (Social Work, Public Health, Healthcare Administration or similar)
  • 4-6+ years of healthcare experience, specifically case management, crisis intervention (or equivalent) and coordination experience (ex: Social Work, Geriatric Case Management)
  • Knowledge of and ability to navigate the healthcare sector and communicate empathetically and thoroughly
  • Experience developing and facilitating care plans
  • Ability to take constructive feedback and innovate in care plan approaches
  • Adaptable and able to work efficiently while managing up to 40 care projects
  • Required to work from a private, secure, distraction-free location with strong internet and phone access
  • Candidates must be located in the US and not reside in excluded states listed
