What you'll do

  • Complete all required tasks according to agency guidelines and policies.
  • Schedule work assignments based on Home Care Worker training and experience.
  • Minimize the volume of open cases to ensure timely resolution.
  • Document all communications accurately in HHA Exchange.
  • Communicate effectively with patients, family members, contractors, clinicians, aides, and other departments regarding issues or changes.
  • Provide necessary information to other healthcare professionals.
  • Consult with family members and other support to ensure ongoing care and that all client needs are identified and met.
  • Advise HHA’s of discharges, changes in service days/hours, or other relevant updates.
  • Complete required daily, weekly, and monthly reports.
  • Manage overtime and travel time requests.
  • Utilize Smart Map and other tools to locate aides, provide directions, and check availability.
  • Complete incident reports.
  • Manage mutual and linked cases.
  • Open and respond to emails in a timely manner.
  • Listen to voicemails and respond promptly.
  • Open and act on faxes in a timely manner.
  • Adapt quickly to agency or industry changes.
  • Discuss any concerns or questions with the supervisor.
  • Any other duty assigned to meet the agency’s needs.
  • Coordinate work assignments based on Home Care Worker training and experience.

  • Document all communications and relevant information accurately in HHA Exchange.
  • Monitor and resolve open cases to ensure timely resolution.
  • Support patients and families by identifying their needs and coordinating appropriate care.
  • Communicate changes in service days/hours, discharges, and other relevant updates to HHAs.
  • Provide necessary information to healthcare professionals and other stakeholders.
  • Complete daily, weekly, and monthly reports.
  • Manage overtime and travel-time requests.
  • Use Smart Map and other tools to locate aides, provide directions, and check availability.
  • Complete and document incident reports.
  • Monitor and respond promptly to emails, voicemails, and faxes.
  • Follow agency guidelines, policies, and procedures.
  • Adapt quickly to changes within the agency and healthcare environment.
  • Escalate concerns or questions to the appropriate supervisor.
  • Perform other duties as assigned to support agency needs.
