About the Role In this pivotal role as an Advanced Practice Provider (NP/PA), you won’t just be part of the care team—you’ll be at its core, ensuring that every patient’s experience is as seamless, precise, and impactful as possible. Working side by side with a physician, health coach, and patient success coordinator, you’ll help create a level of care that feels as personal as it is powerful. Your work is the foundation that allows physicians to focus on what they do best—making the most insightful and impactful medical decisions. You will take complex patient information and transform it into clear, actionable insights that guide care plans. But your role isn’t just about organizing data; it’s about shaping the patient journey. You’ll be actively involved in facilitating and refining medical plans, ensuring that each decision is backed by precision and purpose. Deep passion for Medicine 3.0 and a strong commitment to evidence-based, patient-centered care. A strategic mindset, capable of balancing big-picture medical planning with meticulous attention to detail. Strong knowledge of healthcare operations and regulatory standards (e.g., OSHA, HIPAA, DEA guidelines). The ability to thrive in a fast-moving environment, proactively managing and prioritizing a high volume of time-sensitive tasks. A commitment to excellence—you hold yourself to the highest standard and take pride in refining every detail until it’s just right. Exceptional communication skills, with the ability to work seamlessly across remote and telehealth platforms.
